I, parent or guardian of the above named child do hereby give approval for this child to participate in any and all league activities during the upcoming season. I assume all risks and hazards incidental to such participation, including transportation to and from the activities. I hereby waive, release, absolve, indemnify and agree to hold harmless the local league organization, organizers, sponsors, supervisors, parents, participants or other persons for any claim arising out of an injury to the child. I understand the League insurance is a $250 deductible, secondary pay policy.

I grant permission to managing personnel of league representatives to authorize and obtain medical care from any licensed physician, hospital, or medical clinic should the child become ill or injured while participating in league activities away from home or at other times when neither parent or guardian is available to grant authorization for emergency treatment.

I authorize the use of the child's game and practice photographs to be used on the league's website and any other league marketing materials. I understand that there will not be any compensation for use of images.
